If you are an agency and you need to register with US Airways Vacations, just fill out the Agency Application, W-9 form if you are in the U.S. or a W-8 form if you are outside of the U.S. Once you’ve filled out the forms, just fax them to the New Account Help Desk at 480-693-6016 or mail your forms to: ATTN: New Account Helpdesk, RC-CRO US Airways Vacations 4000 E.
